Our ECM program is a service offered through MediCal to help people who are facing a variety of life and health challenges. Our goal is to support both individuals and families through a coordinated care program. ECM connects you to a Lead Care Coordinator who will be your dedicated support in identifying and meeting life and health challenges.

Victor’s Enhanced Care Management (ECM) Program is a new service offered through MediCal offering to help people who are facing life and health challenges such as:
- Mental, Physical & Behavioral Health issues
- Food insecurities
- Unstable housing
- Transportation challenges
- Employment and education assistance
- Interactions with law enforcement or crisis intervention due to mental health and/or substance use
- Pregnant and new moms
- Transitioning home from hospitalization
- Flexible Community-Based Schedules
HOW IT WORKS
- Families with children or individuals up to the age of 26 can voluntarily choose to participate in Enhanced Care Management (ECM) Program at no cost.
- ECM connects you to a Lead Care Coordinator who will be your dedicated support in identifying and meeting life and health challenges.
- The Lead Care Coordinator will meet with you in the community or at their agency office.
WHO CAN PARTICIPATE?
- Families with children, teens, or young adults through 26 years old, who have been involved in the Foster Care System.
- Families with children, teens, or young adults enrolled in CCS/CCS Whole Child Model.
- Children and Families at risk of or experiencing homelessness.
- Families with children who have mental health or substance use challenges.
- Families with children transitioning from hospitalization.
- Individuals who are pregnant or postpartum.
- Children, Teens and Young Adults who have been involved with the justice system.
